The Medical Social Worker is a member of the Interdisciplinary Team (IDT) and is responsible for the implementation of standards of care for social work services for Signature Hospice. The medical social worker will be dedicated to the Mission, Values and Care Commitments of SIGNATURE HOSPICE and embrace and demonstrate The Little Things That Make A Big Difference. Assesses and evaluates the psychosocial status of patients / families / caregivers related to the patient’s terminal illness and environment. Provides an assessment in the patient’s identified residence, and assists with a plan, as appropriate, when this is no longer safe. Participates in the development of the plan of care and assesses and evaluates patient’s status by initiating and documenting a plan of care, regularly re-evaluating patient / family /caregiver needs, and revising the plan of care as necessary. Teaches the patient / family / caregiver coping and self-care techniques as appropriate. Recognizes and utilizes opportunities for educating patients / families / caregivers to meet plan of care goals. Works in concert with the other hospice IDT Members. Provides information and referral services for organization and patients / families /caregivers regarding practical and environmental needs. Serves as liaison and support between patients / families / caregivers and community agencies. Assists physician and other team members in understanding significant social and emotional factors related to health problems and death / dying issues. Assists the patient / family / caregiver and IDT members in providing continuity of care. A graduate of a bachelor’s program in social work accredited by the Council on Social Work Education; Master’s degree in Social Work preferred.

Minimum of one (1) year’s experience in health care, hospice experience preferred. Understands hospice philosophy and issues of death / dying. Demonstrates good verbal and written communication, and organization skills. CPR certification preferred. Has valid driver’s license and automobile insurance in accordance with State and / or organization requirements.
