Searching for a candidate who
Is licensed by the State of Michigan to practice physical or occupational therapy
Has strong experience in home care documentation and visits
Has a flexible schedule and is able to see patients assigned to them within 48 hours
Has reliable transportation and is willing to travel/drive anywhere in the metro Detroit Area
Has strong communication skills and is able to communicate quickly and efficiently to staffing office and various home care clients.
PC skills a bonus, along with access to a PC
Has reliable means of communication either cellphone, fax machine, email, land line

The positions functions include, but not limited to the following:
Assesses patients needs and design and implement rehabilitation programs.
Documents and submits all assessment/evaluation, treatment and revisit notes in a timely manner.
Supervises therapy assistants as per Medicare Standards.
Consults and communicates with medical and professional staff from associated home care agencies to plan and coordinate joint patient management objectives.
Conducts staff conferences and plans training programs to maintain proficient staff in therapy techniques and use of new methods and equipment to meet patients needs.
Participates in recruitment, orientation and performance management of staff
Represents advocates and promotes the discipline throughout the agency and encourage broad staff participation.
