Summary:
Under the administrative direction of the Program Director, provides and supervises the treatment of Adult Day Health Care participants; acts as a consultant to the Program Director and is a member of the Multidisciplinary Team. Must work the minimum number of hours to meet the needs of the participants.
Primary Duties and Responsibilities:
Evaluates all participants physical condition and functional status. Establishes a realistic goal and sets up treatment plan to administer physical therapy techniques as prescribed by the referring physician. Completes signed and dated quarterly progress notes, and six-month assessments on all participants receiving Physical Therapy.
Provide Physical Therapy treatment. Instructs and demonstrates to family and others the physical therapy and rehabilitation procedures indicated.
Provides training to staff to implement PT maintenance program.
Supervises program aides in implementing PT maintenance program by instruction, observation, and open discussion.
Records on participants chart as services are given. Records initial and final evaluations, makes daily progress report notes, and schedules participants for further treatment.
Works in close cooperation with staff and participants as a member of the Multidisciplinary Team.
Provides a maintenance training program to center staff and periodic evaluation of those receiving training.
Meets the minimum administrative requirements of the position such as attending meetings and maintaining statistical records.
Participates in the discharge plan and initiates the discharge summary.
Requirements:
Must have CPR and First Aid Certifications. Must have a TB clearance within the last 12 months or last 7 days of employment.
Knowledge of principles and procedures relating to physical therapy and how they apply specifically to frail elderly and developmentally disabled.
Experience with the frail elderly, chronically ill persons or those with developmental disabilities.
Skills in assessing and reassessing the functional capabilities of the frail elderly or those with developmentally disabilities; in identifying and establishing the individual treatment programs for physical therapy; in administering the prescribed physical therapy program, in communicating the principles of physical therapy to other staff members, and in maintaining accurate, on-going records of individual therapy programs
Valid license from the Physical Therapy Board of California.
This is a part-time per diem position; flexible hours, fun, easy-going, and patient environment.
