As a Physical Therapist, you will:
Perform physical motor assessments, develop and implement a rehabilitation program to patients in their homes.
Provide therapeutic treatment and instruction to patients in accordance with physician orders to improve and restore strength, coordination, range of motion and function.
Provide hands-on care, case management and evaluation of the care plan, and education of the patient.
Confer with interdisciplinary team of physicians, nurses, social workers, occupational therapists, and home health aides. Participate in development and revision of the Plan of Care Treatment.
Maintains proper therapy records and statistics.
